A satisfying treatment today removing a painful callus under the plantar fifth metatarsal head, with another developing under the plantar first metatarsal head. This patient is a great example of how foot structure directly influences pressure and function. They have a high-arched (cavus) foot, with a high medial arch, lateral arch, and transverse arch. Because of this shape, pressure tends to concentrate on the first and fifth metatarsal heads, while the second, third, and fourth metatarsals carry very little load. Over time, that extra pressure causes the skin to respond by forming callus in those high-load areas. During today’s appointment we carefully reduced the callus to relieve discomfort and improve pressure distribution. To help prevent the callus from building up again, we discussed a few key strategies: • Wearing well-cushioned footwear • Using supportive orthotics to redistribute pressure • Avoiding barefoot walking where possible We’ll review this patient again in 6–8 weeks to reassess pressure areas and ensure the skin is staying healthy.
